Submitting a medical bill to Impact Health Sharing is now easier than ever. Log into your Impact Health Sharing Member Center and navigate to Medical Bills — a new and improved submission process allows members to submit bills with ease, track submissions in one place, receive alerts if additional information is needed, and review past submissions. Providing as much detail as possible on your medical expense form ensures timely processing — missing information can delay the process. The most important document to request from your provider is the CMS 1500 Form, also known as the HCFA Form or UB04 Form. If your provider is unable to supply the form, request an itemized statement containing all required information. For prescription submissions, ask your pharmacy for an itemized summary — information is often listed on your prescription bottle or the written information provided with your prescription.
