Understanding Your Pricing
Annual Household Primary Responsibility Amount (PRA):
The total your household pays each year toward eligible medical bills before additional expenses become eligible for sharing with the member community. Choose from four options to find the level that best fits your needs and budget.
Co-Share:
After your annual PRA is met, eligible medical bills are shared by the community at 90%. You’re responsible for the remaining 10%, called your co-share, up to a maximum of $5,000 per household each year.
Your Monthly Share Amount May Increase Under the Following Circumstances:
Tobacco or Vaping Use
Members who use tobacco or vape will pay a $50 monthly Tobacco Assessment fee.

Annual Wellness Visit & Labs
Shared 100% by the community, every member can get an annual wellness visit and even keep their current doctor, plus up to $150/member, per year in routine labs.
Concierge Service
For annual wellness visits, Impact can handle the payment details directly with your provider in advance of your appointment. A great way to introduce Impact to your primary provider.
Dental & Vision Savings
Reduced costs available immediately to thousands of dentists and eye doctors nationwide.
