How do I use the annual lab credit?

Each Impact member is eligible for a $150 annual lab credit to use with their annual wellness visit. This credit can be applied to a defined list of preventive lab tests and is shared at 100%, not subject to the PRA or co-share.

What labs are included?

The following labs are eligible under the annual credit:

• Complete Blood Count with Differential and Platelets
• Comprehensive Metabolic Panel
• Lipid Profile with Lipoprotein Particle Assessment
• Hemoglobin A1C
• Vitamin D-25 OH
• C-Reactive Protein
• Fecal Occult Blood Test
• Pap Smear
• PSA (Prostate-Specific Antigen)

Only these labs are eligible under the $150 credit. All other labs follow standard Guidelines and subject to the PRA and co-share.

How to use the credit:

1. Schedule your annual wellness visit
2. Have your provider order the labs from the approved list
3. Choose your lab provider:
   • Use Impact’s preferred lab partner for bundled pricing
   • Or use another lab of your choice
4. If using a lab outside the preferred partner:
   • Pay the provider/lab
   • Submit your receipt through the Submit Bills option in the My Medical Bills section of the Member Center

Reimbursements will apply to the $150 credit, and anything over that may be subject to standard sharing rules.
