The following information must be provided when applying for membership with Impact.

Please have all of the information below available:

• Personal data for all members.
  • Name, date of birth, and medical history.
• Primary Member and participating spouse's identification.
  • Social Security numbers.
  • Government-issued ID numbers (e.g., Driver's License).
• Primary Member contact information.
  • Email address and phone number.
