How do I apply?
Applying for Impact is easy! Let's get started:
Before applying for membership, you should familiarize yourself with the Impact Health Sharing Guidelines.
Please have all of the information below available when you're ready to apply:
- Personal data for all members.
- Name, date of birth, and medical history.
- Primary Member and participating spouse's identification.
- Social Security numbers.
- Government-issued ID numbers (e.g., Driver's License).
- Primary Member contact information.
- Email address and phone number.
- Banking information, including bank routing number and account number, to pay your application fee and first Monthly Share payment.
