How do I apply?

Applying for Impact is easy! Let's get started:

Before applying for membership, you should familiarize yourself with the Impact Health Sharing Guidelines.

Please have all of the information below available when you're ready to apply:

  • Personal data for all members.
    • Name, date of birth, and medical history.
  • Primary Member and participating spouse's identification.
    • Social Security numbers.
    • Government-issued ID numbers (e.g., Driver's License).
  • Primary Member contact information.
    • Email address and phone number.
  • Banking information, including bank routing number and account number, to pay your application fee and first Monthly Share payment.
