There are two ways to register your leads with us.

First, if you have a single person/family that you want to refer, simply fill out the form on this page.

Second, if you have a list of people you want to refer, we've created an Excel template you can download, fill out, and upload back to us. We will reach out to each person and educate them on Impact Health Sharing and give them the tools they need to make a decision on whether Impact is right for their family.
